Visual Material Program Rental

Instructions 
This agreement must be signed by the person responsible for individual or chapter viewing of the named visual material program. Please read the agreement, fill in the required information, and sign it. Two checks should be made out to ANG: The first one for $11.50 to cover postage and handling, and the second check for $20 as a deposit. The second check will be returned upon safe and timely return of the slide show. The completed and signed agreement, along with the two checks, should be sent to:

As the person responsible for individual or chapter viewing of the visual material listed below, I certify that:

• I understand that the visual material program and any accompanying educational materials are copyrighted, and I will abide by copyright laws. My obligations include but are not limited to the following:

  • I will not copy the visual material.
  • I will retain possession of the slide program and educational materials during the rental period, and I will not loan them to another party.
  • I will view the visual material in a machine in good working condition, so no damage occurs to the visual material program.
  • I or the chapter to which I belong will not charge for viewing of the visual material program.
  • I understand that the visual material program and any associated educational materials have left ANG’s possession in good condition. In case of either visible or concealed damage upon arrival, I agree to notify the ANG Visual Materials Development Chair within five days, and I agree to keep the shipping carton and all the packing material until the shipping company inspects the damage claim.
  • I am responsible for all damage to or loss of the visual material program and accompanying educational materials once I receive them. In the event of loss or damage, I agree to pay for replacement of the visual material program and any accompanying educational materials.
  • I will return the rented visual material program and accompanying educational materials so that they are in the hands of the Visual Materials Development Chair on or before the due date, which is four weeks after the viewing date listed below. I understand that failure to return the visual material program and accompanying materials on time will result in forfeiture of the $20 deposit.
  • I will return the visual material program and accompanying educational materials with shipping costs prepaid. I will also insure at the rate of $90 for a one-carousel program, $200 for a two-carousel program, or $60 for a videotape/DVD. I will return them in the original shipping container, unless it was damaged upon arrival.

Please allow at least six weeks between your request and desired viewing date to allow time for processing and mailing. Registration fees for all ANG distance learning programs are nonrefundable.
